Three numbers, one of which is doing the work
Every twenty minutes of screen work, look at something about twenty feet away for twenty seconds. That is the whole rule, and it is probably the most repeated piece of advice in office life.
It is also widely misunderstood in both directions. Some people treat it as a clinical prescription where the numbers were derived from research. Others notice that the numbers are suspiciously round, conclude the whole thing is folklore, and ignore it. Both are wrong in the same way: they are arguing about the numbers instead of the mechanism.
The three twenties are a mnemonic. They were chosen because they are memorable, not because twenty minutes was measured against nineteen or twenty-two and won. What sits underneath them is real, though, and worth understanding — because once you know which part of the rule is doing the work, you can keep the useful part and stop worrying about the stopwatch.
Where the rule comes from
The rule is generally credited to the optometrist Jeffrey Anshel, and it spread through the profession rather than through a trial — the American Optometric Association and similar bodies promote it as a rule of thumb for people who work at screens. It was designed to be repeatable by a patient after one hearing, which explains the shape of it. A guideline of "look 6.1 metres away every 18 minutes for 22 seconds" would be better science and would be forgotten before anyone left the room.
Two things happen to your eyes during sustained screen work, and the rule targets both.
Your focusing muscles hold one position. Focusing on something close requires the ciliary muscle inside the eye to contract, and it stays contracted the whole time you are reading a screen at arm's length. Before screens, ordinary work mixed near and far constantly — a page, then a colleague, then a window. That variation let the muscle release regularly. A screen removes it.
You blink less, and less completely. Blink rate drops noticeably during focused screen work, and the blinks that remain are more often partial. Blinking is what spreads the tear film across the surface of the eye, so fewer and shallower blinks leave that surface less evenly covered, which is a large part of the gritty, tired feeling by late afternoon.
The twenty-foot part is the least arbitrary of the three numbers. Around six metres is conventionally treated as optical infinity: at that distance the focusing muscle is essentially at rest. Look at a plant on your desk instead and you are still focusing, just slightly less hard. That is the difference between a break and a change of subject.
What the evidence actually supports
Here is where honesty is more useful than enthusiasm.
Studies of short, regular screen breaks generally find improvement in how people report feeling — less dryness, less tiredness, less of that end-of-day heaviness. That is a real result and it is the outcome most people care about. What the evidence does not support is the stronger claim the rule often gets dressed in: there is no good basis for saying it prevents any eye condition, changes your prescription, or protects your sight in the long run. One trial of the rule found the expected improvement in how people reported feeling, but noted that a two-week window was not enough to show much change in objective measures at all.
There is also a compliance problem that rarely gets mentioned. The rule only works if you do it, and studies that actually monitor adherence find people are far worse at keeping to it than they believe. A rule followed three times a day is not the rule; it is a rounding error.
So the fair summary is narrow but genuinely useful: regular distance breaks make screen work more comfortable, and comfort is the whole claim. Anyone selling it as eye protection has gone beyond what is known.
One clarification that follows directly from the mechanism: closing your eyes for twenty seconds is not a substitute. It rests the visual input and it does help the tear film, but the focusing muscle does not fully relax just because the lids are shut. How to keep it without another app
The obvious way to run a screen-break rule is a screen notification, and there is something faintly absurd about that. Notifications also arrive without regard to what you are doing, which is why the timer app people install in January is muted by February.
Anchoring works better than alarms. Attach the break to something that already happens in your work rather than to the clock:
- Every time you send an email, look up and out before you start the next thing.
- At the end of a meeting, before opening the next window.
- Whenever you finish a section, a function, a paragraph, a row — whatever your unit of work is.
- Every time you take a drink.
None of these lands exactly on twenty minutes, which is fine, because twenty minutes was never precise. What they do is happen reliably without anything having to interrupt you.
Then reduce the friction to nearly zero. If you can arrange your desk so a window or a long sightline across the room is already in your peripheral vision, the break costs you a glance rather than a decision. A distant view to the side of your monitor is worth more than any reminder system, because the hardest part of the rule is not remembering it — it is that looking away feels like stopping work.
If you do want a prompt, an analog one has a real advantage: a kitchen timer or a small hourglass on the desk marks time without adding another thing on the screen you are trying to look away from.
The setup matters as much as the breaks
A break every twenty minutes will not rescue a workstation that is fighting you for the other nineteen. Three things do more for how your eyes feel at 5 pm than the rule does.
Screen position. Top of the screen a few centimetres below your seated eye height, so your gaze drops slightly instead of climbing. Distance about an arm's length — 50 to 70 cm — and further away is fine if the text is still comfortable. Glare. The eye works harder to read a screen with reflections on it. A window directly behind the monitor or directly behind you both cause problems; side-on light is easiest. Bright overhead lighting reflecting off a glossy panel is the common office version.
Brightness matching. Set the screen to roughly the brightness of the room around it. A display much brighter than its surroundings in a dim room, or much dimmer in a bright one, forces constant adjustment. Text size belongs in the same category — if you are leaning in to read, make the text bigger rather than moving your face closer.
Deliberate full blinks are worth adding to the breaks themselves: closing the lids completely for a moment, a few times, rather than the shallow flicker that screen work produces. It costs nothing and it addresses the half of the problem that looking into the distance does not.
What this cannot tell you
- The numbers are not optimised. Twenty, twenty and twenty were chosen to be memorable. Nothing establishes that they beat fifteen or thirty, and treating them as precise misses the point of the rule.
- It is about comfort, not protection. There is no good evidence that screen breaks prevent eye conditions or change your eyesight. Comfort during and after screen work is the honest claim.
- It cannot fix a bad setup. Glare, a screen at the wrong height, and text too small for the distance will each outweigh the breaks.
- It cannot tell you whether your eyes need testing. Discomfort at a screen is sometimes an uncorrected prescription doing exactly what an uncorrected prescription does. A rule of thumb is not a substitute for a proper eye examination, and persistent trouble with vision is a matter for a qualified professional.

Does the 20-20-20 rule actually work?
For comfort, the evidence is reasonably supportive: people who take regular distance breaks report less dryness and tiredness during screen work. For anything stronger — protecting your sight, preventing conditions, changing your prescription — there is no good evidence, and claims in that direction go beyond what is known. Treat it as a comfort habit that costs nothing.
Can I look at my phone during the break?
No, and this is the most common way the rule gets accidentally cancelled. A phone sits at roughly the same distance as your monitor, so your focusing muscles stay contracted throughout what you thought was a break. The distance is the active ingredient. A window, a corridor, or a far wall all work; a screen at arm's length does not.
What if there is no window and nothing twenty feet away?
Use the longest sightline you have and accept a partial benefit — the further the better, and the relationship is gradual rather than a cliff at six metres. A door opened onto a corridor, a diagonal across the room, the view down a stairwell. If you are working in a genuinely small space, this is a good reason to take some breaks on your feet and step outside the room entirely.
Do blue-light glasses do the same job?
They address a different claim, and the evidence for that claim is weak — reviews of blue-light filtering lenses have generally not found meaningful improvement in screen-related eye discomfort. The mechanisms the 20-20-20 rule targets, sustained near focus and reduced blinking, are not affected by a filter at all.
