The trial that complicated "a calorie is a calorie"

In 2019, NIH researcher Kevin Hall and colleagues published a controlled trial in Cell Metabolism that put the same group of adults through two weeks eating an ultra-processed diet and two weeks eating a minimally processed one, with each diet matched for calories, sugar, fat, fibre and macronutrients on paper. The two diets were designed to look equivalent on a nutrition label.

They did not behave equivalently. On the ultra-processed diet, participants spontaneously ate about 500 more calories a day and gained roughly a kilogram over two weeks; on the minimally processed diet, they ate less than they were offered and lost a comparable amount. Nobody was told to eat more or less — the diets were simply available, and people ate to a different stopping point on each one.

What processing actually changes

Ultra-processed foods tend to be energy-dense, low in fibre, and engineered for palatability — texture and flavour combinations that are easy to eat quickly and past the point of feeling full. Whole and minimally processed foods usually work the other way: more fibre and structure means more chewing, slower eating, and a fullness signal that arrives closer to when you actually stop needing more food. None of this means every processed food behaves identically. Reading the difference without a lab

The single most practical filter is the ingredient list rather than the calorie count.
